El Paso County Public Health HCP care coordinators, help examine services and resources that can be put into place to improve outcomes for children and families. HCP is staffed by a multi-disciplinary team consisting of a nurse, social worker, and community resource specialist. HCP care coordinators have the capacity to provide an extra level of support that is individualized to the unique goals and barriers that each family is facing. HCP care coordinators provide resources to empower the family to advocate for themselves and their child and can help walk families through the process of identifying and prioritizing goals. An important aspect of HCP is that we are not limited by insurance because our services are free and there is no eligibility requirement related to income or diagnosis. HCP is voluntary, does not provide direct services and is not a home visitation program.

HCP Program:

  • Provides information, support, resources and referrals for children and youth who have, or are at increased risk for a physical, developmental, behavioral, or emotional condition beyond that of typically developing children
  • Serves children from birth to 21 years of age living in El Paso County
  • Offers Information Only or on-going Care Coordination which provides an additional level of support
  • Provides families the resources and knowledge to manage a wide range of questions, concerns, and services for their child
  • Equips and empowers families to feel confident coordinating their child’s health care and navigating the complex medical care system
  • Supports families with identifying school needs such as providing information for school advocacy, identifying appropriate contacts, and providing information regarding learning options
  • Examines the medical, behavioral, and social factors that a child/family are facing
  • Helps mitigate barriers to accessing necessary resources
  • Assists with linkage to care, navigating referrals and finding specialty clinics
  • Promotes communication among the care team and works with community partners to increase and strengthen supports for families
  • Works with the family to advocate for the child, identify and prioritize needs, and develop a plan to achieve goals

HCP can link families to resources for…Food, Transportation, Financial/Employment, Health Insurance, Rent/Housing/Utilities, Child Care/Respite, Primary and Behavioral Health Care, Medication, Medical Equipment, and Dental Care
